Client: Spero Stuttering

A nonprofit organization that helps, empowers, and advocates for the stuttering community and their families.

  • For Lauren Schlemmer, her experience as a person who stutters greatly influences her response to clients as an SLP who stutters. And it starts with grace for herself.

Lauren was patterned to see her stutter as less desirable than fluency, and it led to her having negative reactions, as well as a negative self-image. Today, even with years of resetting negative self-talk, Lauren finds herself falling back into some of these thoughts at times.

This is where grace plays an important role for people who stutter. We are all human, so Lauren gives herself grace when she stumbles into negative thoughts.

Lauren encourages others to have grace for themselves as well. The process isn’t always straightforward, but Lauren’s own experience helps guide others as they discover their authentic voices.

In our special event, “The Value of Stuttered Voices in Speech-Language Pathology,” Lauren digs deeper into her experience, alongside five more SLPs who stutter, giving a unique look into the lives of people who stutter and how their stutters have made a difference in their approaches to therapy.

  • Randy Panzarino’s therapy today focuses on overcoming shame, which he feels is a large part of why people who stutter turn to hiding and concealing their stutter.

But Randy believes that one of the best ways to overcome shame is to become more vulnerable, a belief he learned from Brene Brown. The vulnerability we show is a measure of our courage, as we show our authentic self to others.

And his students know that his therapy room is a safe place for them to be authentic, honest, and vulnerable. Through that open and honest communication, they can begin building courage to be vulnerable beyond those walls.

Hear more from Randy and five more SLPs who stutter in “The Value of Stuttered Voices in Speech-Language Pathology,” a special event Spero held this past May to celebrate Extraordinary Ordinary SLPs Who Stutter during National Stuttering Awareness Week.

  • Did you know?

Stuttering is a neurophysiological condition, meaning that it can have both physical and cognitive effects due to the interconnectedness of the brain and the nervous system’s functioning.

    Physical Effects:

    ⚠️ Avoidance behaviors

    ⚠️ Negative reactions to speaking

    ⚠️ Escape behaviors (secondary mannerisms)

    ⚠️ Physical tension

    ⚠️ Self-imposed silence

    Cognitive Effects:

    ⚠️ Fear

    ⚠️ Low self-confidence

    ⚠️ Hopelessness about the future

    ⚠️ Social anxiety

    ⚠️ Mind consumed with stuttering

This communication difference can be impacted by a variety of factors:

    ▪️Genetics

    ▪️Hereditary Factors

    ▪️Language

    ▪️Motor

    ▪️Temperament

    ▪️Environment

While research is still being conducted, studies have shown that neurodevelopmental differences in the speech centers of the brain may play a role in stuttering and other fluency conditions. This contradicts much of the thought that brought about various ineffective (and sometimes damaging) therapies in the past.

With this new research, we now know that the route to improved quality of life starts with:

    ▪️Acceptance

    ▪️Social support

    ▪️A feeling of safety

▪️The ability to speak freely and openly

    ▪️Emphasizing one’s strengths and possibilities

As SLPs, we must work to break the stigma around stuttering through empathy, advocacy, allyship, and innovation.

Help us change the paradigm by:

▪️Staying up-to-date with the latest in stuttering-related speech-language pathology news (join our mailing list)

▪️Helping SLPs find educational opportunities to further their understanding of and relationships with people who stutter (make a donation)

▪️Starting your own journey as an SLP with courses, events, and community gatherings that focus on people who stutter and how to support and advocate for them (become an Ally of Stuttering™)

  • 📣 New Professional Development Opportunity!

Stuttering can create challenges with communication, but many of those challenges are more deeply rooted in social stigma, prejudices, and disabling responses to stuttering.

But by reframing stuttering through the social model lens of human diversity, people who stutter can experience improved quality of life.

If you’re ready to dive deeper into the social model of diversity, we have a new course for you!

Northern Speech Services has a new online course led by Spero president Ana Paula Mumy, SLPD, CCC-SLP: “Incorporating the Social Model of Diversity in Our Therapy Frameworks with People Who Stutter.”

This deep dive into the social model of diversity will help SLPs better understand how to approach stuttering in ways that minimize guilt while promoting better self-advocacy and empowerment for people who stutter. Through targeted person-centered strategies, this course helps SLPs empower individuals who stutter to explore the social, emotional, and psychological aspects of their stuttering.

    This course covers:

✅ Medical vs. Social Model of Treatment

    ✅ Barriers Related to Stuttering

✅ Minimizing Structural, Environmental, and Attitudinal Barriers

✅ Possible Goal Areas in Treatment

    ✅ Stuttering Resources

    James Earl Jones.
    Emily Blunt.

They’re all blockbuster stars. And they’re also all people who stutter.

In a recent interview, reporter and person who stutters John Stossel sat down with actor Emily Blunt to discuss stuttering and how it affected their lives—both as children and as adults.

For Emily, it was hard to overcome the embarrassment and fear she felt growing up. But she sums up one of the most powerful strategies in just a few short words:

“Don’t be ashamed. Everyone’s got something—that’s just our thing.”

This candid conversation talks about the need for de-stigmatizing stuttering, how “distractors” offer only temporary fixes, and how all the tricks that have been perpetuated in the past often only cause more struggle and less communication freedom for individuals who stutter.

  • Today, we’re starting a 4-part series on approaching stuttering therapy with The Social Model Lens of Human Diversity.

One of the most important shifts for people who stutter is alleviating blame or guilt for their stutter and reframing the cognitive view of the self and the problem.

To put it simply, many people who stutter need to hear that “it’s okay to stutter.”

    💜 💜 💜

A few principles and actions to keep in mind:

    1️⃣ Stuttering is Okay

Stuttering is not disabling, but social constraints that demand fluency are. People who stutter need to know that their authentic voices are acceptable in all situations.

    2️⃣ Alleviate Blame and Guilt

As a neurophysiological condition, stuttering is not within the control of the individual. They cannot change their neurological makeup any more than someone with diabetes can will their blood sugar into acceptable constraints.

    3️⃣ Embrace Stuttering

Individuals who move toward rather than away from the problem often find that their outward symptoms become less frequent and less struggled. Accepting the stutter as part of their authentic selves can reduce anxiety and fear related to showing their stutter in front of others.

4️⃣ Restructure the View of the Self and the Problem

Inward change can bring outward change. By reframing an individual’s self-image to eliminate stigmas and self-limiting beliefs, people who stutter can communicate more freely and effectively.

  • Over the next several weeks, we’ll be breaking down the 5 focus areas from our short film, “Nurturing and Empowering Your Child Who Stutters,” our newest caregiver resource!

    FOCUS AREA 1: Communication 🗣️

Many of the strategies children who stutter are taught to use may inadvertently encourage attempts to hide stuttering. Monitoring one’s own speech at a young age is difficult, and in many cases it can lead to less talking and more avoidance.

But as parents, we can show the value of our children’s voices by encouraging them to speak their ideas freely and without fear of stuttering. We have a unique position as parents to provide a safe and welcoming environment for our children to be themselves, whether or not they stutter.

Here’s how you can best support your child’s COMMUNICATION:

1️⃣ Focus on your child’s message and ideas. Be a patient and welcoming listener for your child, including when they stutter.

2️⃣ Let communication be about connection, not fluency or performance.

3️⃣ Create safe spaces where your child can stutter without time pressure or judgment. Spending one-on-one quality time with your child daily can help them feel heard and valued.

Want to learn more ways to support, nurture, and empower your child who stutters through evidence-based approaches?

Our 20-minute short film guides you through five core focus areas that will guide you in ways to build your child’s resilience, confidence, and joy in communication.

  • As SLPs demonstrate the necessity and value of the services we provide for people who stutter, we must look beyond percentages of stuttered speech as the primary measure of "severity" and/or progress.

We can begin by using the ICF model on the experience of stuttering (Tichenor & Yaruss, 2019) to demonstrate the totality of stuttering, and we can demonstrate progress based on assessment measures related to increased life participation, improved quality of life, and reduced negative impact.

In other words, there are many ways to demonstrate "severity" of stuttering based on affective and cognitive factors as well as tangible growth and progress that are unrelated to visible stuttering behaviors (or “quantifying” stutters). Additionally, no single measure or assessment should be used as the sole criterion to determine eligibility and need for speech services.

Need ideas on assessment tools in order to evaluate stuttering holistically?

Here are some options that may be considered, depending on the individual’s age and circumstances, which dive into impact, participation, communication apprehension, stuttering anticipation, emotional reactions and attitudes, coping responses, and quality of life.

☑️ Overall Assessment of the Speaker's Experience of Stuttering (OASES)

☑️ Personal Report of Communication Apprehension (PRCA)

☑️ Palin Parent Rating Scale (Palin PRS)

☑️ Observational Rating Scales of the Test of Childhood Stuttering (TOCS)

    ☑️ Stuttering Anticipation Scale (SAS)
Speech Situation Checklists (SSC-ER and SSC-SD) of the Behavioral Assessment Battery for School-Age Children Who Stutter (BAB)

☑️ Behavior Checklist (BCL) of the BAB

☑️ Communication Attitude Test (CAT) of the BAB

    ☑️ Erickson S24 Scale

    ☑️ Self-Efficacy Scales

☑️ Stuttering Pride & Prejudice Scale

    ☑️ Classroom Observations

    ☑️ Teacher Questionnaires

  • What drives you toward excellence?

Is it the need for recognition?

    Is it money?

    Power?

    Jealousy?

    Selfish Ambition?

None of these things tip the scales when you’re confident. Excellence isn’t a badge to be earned but an ideal to strive towards. You’re not doing this to prove to someone else that you’re better, stronger, or faster.

You strive for excellence because excellence represents integrity.

And that integrity, that confidence, starts with how you see yourself.

So, be intentional in what you do and how you do it. Show the people who see you that you are there for more than accolades and honors.

There’s no room for jealousy or fear when confidence is your essence. Stand tall and know that your success doesn’t depend on others.

It depends on your desire to be better today than you were yesterday.

  • Long speeches and loud boasts do not make a man strong or confident.

“Walk softly, but carry a big stick.”

When Theodore Roosevelt made this statement over a century ago, it struck a chord of truth:

The most powerful, influential, and admirable men in history often lived by this very mantra. It’s far easier to speak loudly and not back up your claims. It takes a truly great man to speak very little yet hold captive the attention and respect of those he meets.

  • Decorum: (n.) good manners and appropriate etiquette in a given social situation.

It’s seldom we use this word, and it seems it’s seldom we act with it in mind. But today, more than ever, we need a return to decorum.

We need to return to honoring those around us through our actions, words, and choices.

Your Words: be kind with your statements, always demonstrating respect for the people you are with.

Your Actions: remain calm and confident rather than reacting in emotion.

Your Choices: choose your attire to honor the occasion, not to honor your own comfort or vanity.

True decorum defers attention to someone else rather than attracting attention to yourself.

  • How does your style reflect your character?

A dignified man knows that his style can either draw attention from the people around him or give honor to those he encounters.

When you choose to show dignity in your attire, take these steps to make it represent your character:

🔹 Look to tradition and legacy, not trends

🔹 Choose fine quality over expense or flare

🔹 Use accessories to express your personality without drawing extra attention

🔹 Choose strong yet quiet colors over loud patterns

Dignity assures that all people are honored for who they are, not what they can do. And the dignified man always places people he encounters ahead of himself.

  • The personal and social impact of methamphetamine use is growing, and it impacts more people—and more ages—than you may realize.

While fentanyl is getting a lot of media attention right now, and rightly so, methamphetamine addiction is reaching epidemic levels.

In 2020, 0.9% of people 12 or older reported using methamphetamine in the past 12 months, roughly 2.6 million people.

In 2021, approximately 0.2% of students between 8th and 12th grades reported using methamphetamine in the past 12 months.

And in 2020, roughly 23,837 people died from an overdose involving psychostimulants other than cocaine (primarily methamphetamine).

The PharmChek® Drugs of Abuse Sweat Patch, which is worn on the skin for up to 10 days and provides long-term and continuous drug testing, not only acts as a deterrent to this type of drug abuse but is changing the sobriety landscape.

  • A Florida teen was recently arrested for having enough fentanyl to kill over 1,500 people. Here are some alarming facts about illicitly-manufactured fentanyl that more people need to know:

The teen in question was in possession of 3.17 grams. However, drug trafficking organizations usually distribute fentanyl in kilograms, enough to kill half a million people.

Fentanyl is a synthetic opioid that is up to 50 times stronger than heroin and 100 times stronger than morphine.

Every day, over 150 people die from overdoses of synthetic opioids like fentanyl.

Fentanyl is available on the drug market in both liquid and powder forms. Powdered fentanyl looks like many other drugs and is frequently mixed with heroin, cocaine, and methamphetamine. In liquid form, fentanyl can be found in nasal sprays, eye drops, and dropped onto paper or candy. Most people who purchase illegal drugs have no idea that their drugs are laced with fentanyl.

On the street, fentanyl can go by many names, including Apache, China Girl, China Town, China White, Dance Fever, Great Bear, King Ivory, Friend, He-Man, Goodfellas, Jackpot, Murder 8, or Tango & Cash.

Recognizing signs of opioid overdose can save a life. Here are things to watch for:

    ⚠️ Constricted pupils

    ⚠️ Loss of consciousness

⚠️ Slow, weak, or no breathing

    ⚠️ Choking or gurgling sounds

    ⚠️ Limp body

    ⚠️ Cold or clammy skin

    ⚠️ Discolored skin

The need for long-term and continuous monitoring and testing of fentanyl has never been more evident, and the PharmChek® Sweat Patch is a valuable tool in fighting the opioid crisis.

  • After thirty years of testing and analysis, we have rarely encountered a person who wasn’t a good candidate for the PharmChek® Sweat Patch. Here’s why:

While the upper arm is the ideal placement location for the sweat patch, it’s not the only place where the patch can be worn.

If someone’s upper arm is too hairy or has a skin abrasion, the sweat patch can also be applied to the lower back or lower midriff. Between these three areas as suitable locations for wear, very rarely is someone not a good candidate for the sweat patch.

The PharmChek® Sweat Patch is proven to be the better, cleaner, and smarter option to detect marijuana, cocaine, opiates, amphetamine, methamphetamine, PCP, fentanyl, oxycodone, oxymorphone, hydrocodone, and hydromorphone.

  • How do we know that results from the PharmChek® Sweat Patch are so reliable?

For starters, its thirty-year track record of providing distinct advantages over other forms of drug testing such as urine, saliva, or hair, makes for a compelling argument. But that’s not all:

    It’s cleared by the FDA.

Dozens of peer-reviewed academic studies support its accuracy and reliability.

Hundreds of courts have ruled its results as reliable and admissible.

All results are promptly reported to supervising authorities via a secure, password-protected web portal.

It’s true there are many options available when it comes to drug testing, but only one that is tamper-evident, provides continuous monitoring, is a hygienic alternative to UA, and is proven in court time and time again.

The PharmChek® Sweat Patch is far and away the better, cleaner, and smarter option to detect drugs of abuse.

  • What pivotal role can medication-assisted treatment (MAT) play in drug courts?

While short-term cases may not always require MAT, chronic substance use often demands intervention for safe detoxification and successful treatment.

MAT becomes essential for chronic users due to the buildup of chemicals in the body, uncontrollable cravings, and severe withdrawal symptoms, especially with substances like stimulants and opioids.

The non-invasive nature and continuous monitoring capabilities of PharmChek® make it an ideal tool for overseeing MAT progress and adjusting treatment strategies as needed.

Additionally, its integration with drug court programs facilitates seamless communication between treatment providers, court officials, and participants, promoting accountability and enhancing overall treatment outcomes.

  • How do we combat BZD abuse among juveniles? By focusing on security, support, and accountability.

🔒 BZD abuse is often connected to prescription accessibility. Whether from parent prescriptions or their own, juvenile benzodiazepine abuse can be prevented when these substances are properly secured and monitored.

🫶🏼 Addressing juvenile BZD abuse takes careful support and consistent accountability. Teens who need to recover from addiction will need oversight and potentially even medical assistance to endure the withdrawal symptoms and effects, especially if they are chronic users. It will also take consistent efforts for court officials to help maintain sobriety and avoid relapse.

🔍 While observation is a major part of this accountability, drug courts will likely need methods like continuous drug monitoring to prove sobriety and provide feedback for improved treatment techniques.
